  • Submit the authorisation form, a photo ID, and a copy of the health card at the desk.
  • During this process, the hospital will forward the authorisation form and other necessary medical details to Reliance company or TPA through mail or fax.
  • Meanwhile, the claimant/insured person will sign each and every medical bill.
  • Make sure you leave the original medical investigation reports and discharge papers with the hospital authority.
  • You can keep a photocopy of the same with you for records.

The above process is meant for planned hospitalisation. In case the patient requires emergency hospitalisation, the procedure will be a bit different. First things first, get the patient admitted to a network hospital. Next, get the pre-authorisation form filled out and signed by the doctor who is attending the patient. The rest of the procedure will be the same.
